At the other end, MaineCare's outpatient therapy rate on H0004 works out to $43.88 an hour, well under the psychotherapy median. Maine also attaches heavy prior-authorization rules to its behavioral health codes. The combination makes MaineCare one of the most rewarding and most closely managed behavioral health payers in the country.

H2019 Maine Medicaid rate: first in the nation

MaineCare's H2019 rate took effect January 1, 2026. The gap to the next state is wide: Wyoming pays $149.80 an hour, Washington $145.40, Idaho $142.44 and Ohio $128.96. The lowest states are Wisconsin ($21.44), Arizona ($19.52) and Alabama ($16.80).

A rate this far above every other state typically reflects a more intensive service definition than a basic hourly H2019 code. The economics are still striking. The Maine median psychiatric technician wage is $21.79 an hour, close to the $21.70 national figure. That wage is 6.9% of the H2019 hourly rate, leaving a margin of $418.75 an hour after a loaded wage. Even after supervision, clinical oversight and program overhead, the rate leaves substantial room.

What the HE modifier means for billing

The rate is tied to the HE modifier, which marks a mental health program. The same base code with a different modifier can carry a different MaineCare rate, so a provider should confirm that its service, staffing and program enrollment match the rate line before budgeting at this level. How MaineCare organizes behavioral health payment

MaineCare publishes rates through numbered sections of the MaineCare Benefits Manual. Physician services sit under Section 90, advanced practice nursing under Section 14 and private non-medical institutions under Section 97, with Appendix E covering mental health facilities. A behavioral health agency may bill under more than one section depending on the service and setting.

MaineCare's psychotherapy reference rate tells the opposite story from H2019. It pays $10.97 per 15 minutes on H0004 with modifiers HN and HQ, or $43.88 an hour, effective January 1, 2026. The national median for outpatient psychotherapy and diagnostic evaluation is $83.33.

With a Maine median counselor wage of $28.83, the wage takes 93.4% of the H0004 hourly rate, leaving a margin of $2.90 an hour after a loaded wage. That margin cannot cover documentation, no-shows or supervision. For a Maine outpatient clinic, this code works only when delivered efficiently or alongside better-paid services.

How much does MaineCare pay for 90837?

The CPT session codes paint a different picture from H0004. The hour-long psychotherapy session, code 90837, pays $118.41, effective January 1, 2026. That ranks 28th of 48 states against a $132.38 national median, in the lower-middle of the national range rather than near the bottom.

The gap between the CPT codes and H0004 reflects the modifiers. H0004 with HN and HQ is billed for a specific staff level and setting, while 90837 is billed by licensed clinicians. A practice whose clinicians bill the CPT codes earns far more per hour than one billing H0004 under that modifier combination. The assessment rate deserves attention alongside H2019. At $162.04 an hour, H2000 pays well above the national median for a non-physician assessment, and because it is billed in 15-minute units, a thorough assessment is paid in proportion to the time it takes. That suits programs serving clients with complex histories, where a full assessment can run long. It also means that assessment time should be documented carefully, since each unit is subject to the 47 prior-authorization rules on the code.

Skills training, by contrast, is a modest line. A provider whose program depends on it should treat it as a supporting service rather than a revenue driver, and fund it from the stronger H2019 and assessment lines.
