The New Hampshire Medicaid fee schedule pays $92.06 for code 99213, a level-3 return office visit, in 2026. That ranks 11th of 46 states and sits well above the $75.27 national median. Look one level up, though, and the picture changes: 99214 pays $108.46, just under the median and 24th nationally. Many New Hampshire rates have not moved since 2023, and the schedule mixes top-ten rates with some of the lowest in the country.

Hospital inpatient is unusually large in the dataset at 7,179 rates, second only to physician services (9,324).
Of 26,581 rates, 26,322 sit in New Hampshire Medicaid (fee-for-service, DHHS / NH MMIS). Waiver programs include Choices for Independence (CFI), the Acquired Brain Disorder and Developmental Disabilities waivers, In Home Supports for children with developmental disabilities, and a 1915(i) Supportive Housing benefit. The effective dates show where the split comes from. Code 99213 carries a January 1, 2024 rate, while 99214, 99215, 99203 and 99204 still carry July 1, 2023 rates. The result is that 99215 pays $146.29 (25th), and 99214 pays almost exactly the national median. New-patient visits do better: 99203 pays $109.37 (14th) and 99204 pays $166.45 (15th).
Behavioral health is solidly above the median. Code 90834 pays $100.57 (15th of 47), and 90791 pays $176.03 (ninth of 44), slightly above Medicare's $175.09. Emergency code 99285, at $186.79, is fifth of 48.
Two codes rank near the very bottom. Speech therapy under 92507 pays $19.57, 42nd of 44 states, against a $62.12 median. The adult preventive visit under 99396 pays $44.15, 43rd of 46. Therapy and preventive care providers should not assume the 99213 rank carries across the schedule.
ABA in New Hampshire: technicians paid more than analysts
New Hampshire inverts the usual ABA pricing. Technician 97153 pays $17.79 per 15 minutes ($71.16 an hour), 14th of 42 and 12.2% above the median. But analyst-directed 97155 and assessment code 97151 both pay $16.43 per unit, less than the technician rate, ranking 39th and 40th of 41. Nationally, the 97155 median is $25.10 and the 97151 median is $29.14.

NH Medicaid Care Management: what plans must pay
New Hampshire's managed-care program is Medicaid Care Management (MCM). The named plans are AmeriHealth Caritas New Hampshire, NH Healthy Families and WellSense Health Plan. Adult dental is administered by Delta Dental of New Hampshire, and Coordinated Transportation Solutions brokers transportation for fee-for-service members.
Of 21 service lines classified, 11 carry a published-rate rule:
- Plans must pay at least the published rate for hospital inpatient and for FQHC, RHC and clinic services.
- Plans negotiate, and the published rate applies out of network for physician services, hospital outpatient, behavioral health, lab, radiology, pharmacy, and vision and hearing.
- Paid directly by the state for dental, nursing facilities, home and community-based services and other services.
One state directed payment program took effect July 1, 2026: plans must pay DME and supplies at a minimum of 80% of the DHHS FFS State Plan fee schedule. See the New Hampshire managed care page.
Industry flagship rates in New Hampshire
- Private duty nursing: RN care pays $75.84 an hour, 10th of 43 and 38.3% above the median. See New Hampshire private duty nursing rates.
- Physical therapy: 97110 pays $99.80 an hour, 19th of 49 and 3.9% above the median, at 84% of Medicare. See New Hampshire therapy rates.
- Personal care: T1019 pays $32.00 an hour, 13th of 27 and 4.8% above the median.
- Behavioral health counseling: $27.38 per unit, sixth of 12 and 4.5% above the median.
- Skilled nursing: $271.22 a day, effective July 1, 2026. Hospice: $192.30 a day, effective October 1, 2026.
Recent New Hampshire Medicaid changes
Nursing facility rates dominate the 2026 notices. DHHS published BAAS Appendix A Nursing Facility Rates for January 1, 2026, then revised them in BAAS 26-20 and BAAS 26-27 for July 1, 2026, alongside a Budget Adjustment Factor for July 2026. Other notices include Continuous Recovery Monitoring, Medicaid to Schools CPE and Admin Claiming, and the April 2026 ABA notice.

On a 99213 visit, New Hampshire sits just behind Rhode Island and Massachusetts and well ahead of Connecticut, Vermont and Maine. It leads the region on technician ABA. For personal care, its $8.00 rate trails Rhode Island, Vermont and Maine but sits well above Connecticut and Massachusetts.
