Behavioral health Medicaid rates in Utah were rebased on 2026-07-01, and the result is a schedule that pays above the national median on most community services. Utah Medicaid pays $74.36 an hour for skills training on code H2014, which ranks 9th of 33 states and sits 45.2% above the $51.20 national median. The exception is wraparound: Utah's H2021 rate is the lowest of 11 ranked states and has not been updated since 2018.

Community services after the July 2026 rebase
Utah's community rates leave solid room over wages. The state's median psychiatric technician wage is $19.60 an hour, below the $21.70 national figure.
- Skills training (H2014). $18.59 per 15 minutes, or $74.36 an hour. Wages take 37.5% of the rate, leaving $46.50 an hour after loaded wages. Rhode Island leads this service at $139.64; Idaho pays $8.60.
- Therapeutic behavioral services (H2019). $23.05 per 15 minutes, or $92.20 an hour. Wages take 30.2% of the rate, leaving $64.34 an hour. Maine leads at $449.72, followed by Wyoming at $149.80.
- Service plan development (H0032). $37.51, ranking 8th of 18 and 23.7% above the $30.34 national median. Maryland leads at $131.35.
For a Utah community behavioral health agency, both H2014 and H2019 now cover a median-wage technician with a margin for supervision, travel and documentation.
Wraparound: the lowest rate in the country
H2021 pays $4.94 per 15 minutes, or $19.76 an hour, a rate in effect since 2018-07-01. It ranks 11th of 11 and sits 78.2% below the $90.48 national median. The next-lowest states are New Jersey ($39.08) and North Dakota ($47.36); Louisiana pays $710.72 and Virginia $280.00.
At $19.76 an hour, Utah's wraparound rate is close to its median technician wage of $19.60, leaving almost nothing for supervision, travel or overhead. Unlike the other community codes, it does not carry the 2026-07-01 rebase date.
Crisis therapy and outpatient rates
Utah's outpatient psychotherapy comparison rate is code 90840, a crisis therapy add-on paid at $80.29 per 30 minutes, or $160.58 an hour, effective 2026-07-01. It is not ranked against other states' session rates. At a $30.72 median counselor wage, wages take 27.2% of that hourly rate, leaving $116.92 an hour after loaded wages.
Utah also publishes behavioral health rates under state-specific codes: $75.00 for mental health counseling under code PGE, effective 2025-01-01, and $132.44 for assessment under code APN, effective 2021-07-01. Neither is ranked.
Utah lists no prior authorization rules and no frequency limits on 90785, 90791, 90792, 90832, 90833, 90834, 90836 or 90837.
