Application fee
Federal rule (42 CFR 455.460): the state must collect the CMS application fee from institutional providers that are newly enrolling or re-enrolling; individual physicians and non-physician practitioners are exempt, as are providers already enrolled in (or that already paid the fee to) Medicare or another state's Medicaid/CHIP. Fingerprinting
Florida also requires Level 2 background screening (fingerprint-based, under s. 408.809, F.S.) as part of enrollment and revalidation. Federal rule (42 CFR 455.434, 455.450(c)): providers the state places in the 'high' categorical risk level, and anyone owning 5% or more of such a provider, must submit fingerprints for a criminal background check.
Site visit
AHCA assigns each provider type a limited, moderate or high risk level under 42 CFR 455.450. Federal rule (42 CFR 455.432, 455.450(b)-(c)): the state must do site visits before and after enrollment for providers in the 'moderate' or 'high' risk levels, and any enrolled provider must allow unannounced on-site inspections.
Revalidation
Florida calls revalidation 'renewal': AHCA re-enrolls every provider at least every five years, mailing a notice about 90 days before the provider agreement expires. Claims suspend if the online renewal is not finished by the expiration date. Federal rule (42 CFR 455.414): every enrolled provider must be revalidated at least once every 5 years.

Steps

  1. Use AHCA's Enrollment Readiness Tool to identify the provider type, enrollment category (full, limited, or rendering-only/ROPA), required documents and risk level
  2. Complete Level 2 background screening where required
  3. Apply online through the Enrollment Wizard in the Florida Medicaid Web Portal and upload all forms
  4. Fix any deficiencies within 21 days; AHCA aims to finish processing within 60 days
  5. Receive a nine-digit Medicaid provider number
  6. Get credentialed and contracted by each SMMC plan you want to join (plans must onboard within 60 days of a complete application)

Most Florida Medicaid members are in the Statewide Medicaid Managed Care (SMMC) program across nine regions (A-I). Each SMMC plan credentials and re-credentials its own network under AHCA contract rules: plans must confirm each provider is enrolled with Florida Medicaid and holds a current AHCA provider agreement, and must fully enroll/onboard providers they choose to contract with within 60 days of a complete application (with $5,000 liquidated damages per credentialing failure).
