- Base fee $3,000 per application, plus $5,000 for projects over $500,000 and under $4,000,000; plus $8,000 for $4,000,000 to under $10,000,000; plus $12,000 for $10,000,000 or more. $1,000 for applicant-requested expedited processing; $500 for waiver letters of intent and amendments; $100 per approved covered clinical service on the annual survey (MCL 333.20161).
